History:
56 year old female presented with Chronic pain in the neck, mid and low back as well as severe headaches on a daily basis. Symptoms presented with constant spasms and tenderness throughout her body causing dull, achy to sharp, stabbing pain along with radicular shooting pain into the arm and legs when aggravated. Pain was so severe she was unable to perform her daily activites and She stated she had been to her medical doctor and numerous specialists over the past 5 years with little to no relief.
Examination:
Patient presented in obvious discomfort, with slow guarded movements of the neck and lower back, as well as wide based antalgic gait due to pain levels. Palpation revealed severe tenderness in the paraspinal musculature bilaterally with active trigger points noted in the trapezius, scalenes, rhomboids, quadratus lumborum and piriformis. Spurlings test and Formainal compresion test both elictied severe cervical pain with radicaulr arm pain bilaterally. Kemps, Yeomans and Psoas test elicited severe low back pain and SI joint pain bilaterally.
Diagnosis:
Cervical Neuritis, Thoraic Neuritis, Lumbar Neuritis, Fibromyalgia, muscle spasms and myocitis.
Treatment
Patient was treated with diversified chiropractic adjustments, physiotherapy including stretching strengthening and myofascial release three times weekly for 4 weeks and stated 80% improvement in symptomatology within a 30 day period.
Discussion
Patient suffered with a Fibromyalgia and neuritis that did not respond well to medications or physical therapy and presented to this office as a last hope for pain relief. She was treated for 30 days at which time she stated 80% improvement and that she was able to walk the mall for the first time in 5 years with her family and did not have any pain that evening or the following day. She continues to treat at this office and is able to perform her daily activities
History
50 y/o male presented with chronic low back pain and radicular leg pain for over 10 years causing debilitating pain when aggravated and interfereing with all activities. He was scheduled for lumbar surgery in 6 weeks and was looking for a last resort to prevent sugical intervention
Examination
Patient presented in obvious discomfort with wide based antalgic gait forward lean and favored his right loer back with a (+) minors sign, (+) Lasegue's test at 40 degrees with radicular leg pain. X-rays showed multilevel degeneration with DDD and DJD. MRI showed multiple disc bulges at different level of the lumbar spine.
Diagnosis
lumbar radiculitis, degenerative disc and degenerative joint disease, Lumbar neuritis.
Treatment
Patient was treated with diversified chiropractic adjustments Spinal decompression and an intensive stretching program of the hamstrings, piriformis, and psoas musculature to improive spinal biomechanics and remove abnormal muscle.
Discussion
Patient was treated 2 times a week for 3 weeks at which time his leg pain and low back pain had made a 70% improvement and he was able to perform his daily activities with little to no pain he cancelled his surgery and is now nearly pain free with only occasional discomfort that is controlled with stretching and occasional chiriopractic treatments.
